[plutotv] extractor code cleanup
- url slug processing is now done against the parsed url path so that
   query params don't interfere
 - when trying to get ad free formats we'll skip any duplicate urls

Kevin O'Connor 6605eafcfe
[plutotv] Add a PlutoTV extractor for on-demand content
Supports both movies and series on-demand content. Series can be
downloaded entirely, per season, or per episode depending upon the link
provided.
